Report A Claim

As soon as they are received, claims should be reported to Cox Specialty Markets. Complete the appropriate claims form found below.

Be sure to include:

• Complete contact information for the insured
• Phone numbers and times to contact the insured
• Complete description of the claim and damaged property / parties
• Any suit papers that the insured has been presented with
• Any police reports or descriptions taken from witnesses

Fax or email the claim report along with any other pertinent information to (877) 311-6887 or csm@coxspecialty.com.

Your claim will be documented and forwarded to the appropriate party for handling. We will confirm receipt of the claim and provide you with further information.
