Claims should be reported directly to Cox Specialty Markets. Complete the appropriate claim form found below:

Be sure to include:

• Complete contact information for the insured.
• Phone numbers and times to contact the insured.
• Complete description of the claim and damaged property / parties.
• Any suit papers that the insured has been presented with.
• Any police reports or descriptions taken from witnesses.

Then fax the claim report form along with any other pertinent information to (937) 547-6472.

Your claim will be documented and forwarded to the appropriate party for handling. We will confirm receipt of the claim and provide you with further information.
